Date Last Worked (DLW)

The date you last worked is the last day you earned a paycheck, or the day before you were no longer employed.

The date last worked affects several other factors, such as back pay, date last insured, and perhaps onset date. If your disability is due to an injury and you stopped working the same day as your injury, your date last worked and your onset date would be the same. In cases of illness or more gradual disabilities, your date last worked may be different from your onset date.

If you are unsure of your date last worked, check with your most recent employer—they may have a record on file. If not, try to be as specific as possible when determining your date last worked.
